UGA Extension is currently accepting registration for our Fall 2026 cohorts. Upcoming cohorts are listed below. Space in each cohort is limited and interested participants meeting eligibility requirements will be prioritized. If you would like to enroll in one of these cohorts or remain on our email list for future programs, please complete our online registration form hosted through DPP Express, a secure program management platform from our partners at the American Diabetes Association.
As a reminder, eligibility requirements for this program are:
- Adult aged 18+ with a BMI of 25 or greater (23 or greater for people of Asian descent)
- Not previously diagnosed with type 1 or type 2 diabetes
- High risk of prediabetes determined by blood work from your healthcare provider, a history of gestational diabetes, OR a score of 5 or greater on the prediabetes risk test
